Anti-reflux surgery: who needs it and who decides?

Several indications have been proposed for anti-reflux surgery. These include a patient's need for continuous medical therapy, an incomplete response to therapy, high volume reflux, the presence of respiratory complications, Barrett's esophagus, and finally the patient's own preferences.
